For many years, the weak link in a Total Shoulder Replacement has been the glenoid----concern about glenoid implant loosening has resulted in avoiding a "Total" in younger patients, and in advising against overly heavy lifting etc after the surgery.
The newest type of glenoid design offers some advantages which may ultimately translate into better survivability, and fewer concerns about activity level. Whereas the Keeled type requires cement, the newest Pegged implants typically have a few peripheral pegs--which are fixed with only a dab of cement-- and a larger ridged/slotted central peg which allows for bony ingrowth.
Indeed, 2 recent articles in the Journal of Shoulder and Elbow Surgery suggest that bony ingrowth occur into the central peg, and that lucency around the glenoid implants is less with the pegged design as compared to the keeled design.
